Shriners Children’s is seeking an RN Care Manager to join our pediatric specialty care team in California. The role focuses on advocacy, care coordination, and discharge planning across the care continuum in our renowned burn, orthopaedic, and subspecialties centers.

The position requires a BSN, experience with disease management, and nursing licensure in the state. The pay range offered is $73.05–$103.03 per hour, with compensation tied to experience.

Qualifications

• BSN degree required.
• Experience with disease management.
• Proficiency with motivational interviewing techniques.

Responsabilités

• Advocates for patient and family within the treatment plan and guidelines.
• Collaborates with interdisciplinary team to meet organizational goals.
• Conducts intake assessments at initial visit or by phone before first appointment.
• Ensures coordinated care with Medical Home and other care providers.
• Performs discharge/transition assessments and plans care to support outcomes.
• Assists with medication management and adherence through follow-up.
• Reviews service authorizations to support medical necessity and care level.
