Therapy 2000 is seeking a Pediatric Physical Therapist to support families in Central SW Houston/Almeda with home-based therapy. This role emphasizes compassionate pediatric care, collaboration with families and an opportunity to contribute to a child’s growth and development in the setting where they feel most comfortable.

In this position you will perform evaluations, design and implement treatment plans, develop home exercise programs with caregivers, and coordinate care across providers to

Qualifications

• 3+ years as a pediatric Physical Therapist
• Experience in a home-health setting
• Specialty Certifications (example: PCS)
• Bilingual (English/Spanish)

Responsabilités

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
