Therapy 2000 is seeking a Pediatric Physical Therapist to serve families in the Aliana, Mission Bend, and Telfair areas of Houston, TX. This home-based role involves evaluating children, designing personalized treatment plans, and coordinating care with caregivers and other health professionals.

The position offers a sign-on bonus for therapists with HRI experience, plus benefits and opportunities for ongoing education.

Qualifications

• Pediatric PT license or eligibility required.
• Home-health setting experience preferred.
• Bilingual in English/Spanish is highly valued.

Responsabilités

• Perform comprehensive physical therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with caregivers and coordinate care with other providers.
• Meet patient visit quotas and maintain timely chart updates and documentation.
• Adhere to agency policies, ethics, and relevant home-health regulations.
