Therapy 2000 is seeking a Pediatric Physical Therapist/PTA to join our mission-driven team providing home-based pediatric therapy across Carrollton, Farmer's Branch, and Addison. The role focuses on evaluating needs, designing therapy plans, and enabling children to reach their fullest potential in familiar home environments.

You will collaborate with families and healthcare providers, maintain high standards of care, and participate in ongoing professional development, with a sign-on bonus

Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in a home-health setting.
• Specialty Certifications (PCS).
• Bilingual English/Spanish.

Responsabilités

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with patients' caregivers.
• Collaborate with multidisciplinary teams to coordinate care.
• Meet minimum patient visits and complete chart updates and logs.

Qualifications

What you’ll need:

• 3+ years as a pediatric Physical Therapist
• Experience in a home-health setting

Responsibilities

What you’ll do:

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
