Job summary

Qualifications

• State license to practice Physical Therapy in the relevant state.
• Ability to work with medically fragile preschoolers and families.

Responsibilities

• Provide PT services to children from birth to three as per IFSP plan.
• Conduct assessments and develop treatment plans.
• Document assessments, treatment notes, and reports in secure systems.
• Monitor progress and adjust plans to meet IFSP goals.
• Collaborate with family and other team members for consistency of care.
• Supervise students or interns as needed.
