Therapy 2000 is seeking a Pediatric Physical Therapist/Assistant to join a mission-driven team in Mansfield, TX. This home-based role offers both full-time and part-time opportunities, requiring 3+ years in pediatric PT and experience in home-health settings.

Responsabilidades

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs with patients’ caregivers.
• Coordinate care with other providers and disciplines.
• Complete a minimum number of patient visits weekly as required.
• Maintain timely chart updates, progress summaries and logs.
• Meet with agency personnel to align expectations and standards.
• Comply with agency policies, procedures, and applicable laws.

Qualifications

What you’ll need:

• -3+ years as a pediatric Physical Therapist
• -Experience in a home-health setting

Responsibilities

What you’ll do:

• -Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• -Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• -Develop home exercise programs in partnership with patients’ caregivers.
• -Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• -Complete a minimum number of patient visits on a weekly basis as required by the agency.
• -Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• -Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• -Perform all duties in compliance with agency policies and procedures.
• -Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
