Job summary

Paylocity in Nevada is seeking an experienced Registered Nurse to provide direct palliative care within an approved plan of care for terminally ill patients and families. You will triage emergencies, coordinate with the hospice team, and ensure clear communication of patient needs.

Key duties include following an interdisciplinary care plan, documenting thoroughly, and participating in team meetings. CPR/First Aid and an active RN license are required; on-call hours and travel may be involved.

Qualifications

• Must hold a current RN license in the state of Nevada.
• Prefer hospice/home health or pediatric care experience.
• Background check, pre-employment screening, TB testing.
• Requires read/write/speak English.
• CPR/First Aid certification is required.
• Must be available for on-call hours, weekends, and holidays.

Responsibilities

• Foster and maintain effective relationships with co-workers, pediatric patients, and community professionals.
• Follow the individualized Plan of Care established by the interdisciplinary team.
• Gather, report, and analyze patient care data and adjust care accordingly.
• Coordinate overall care for assigned patients across hospice disciplines during all hours.
• Communicate via proper chain of command and participate in interdisciplinary meetings.
• Provide patient and family education on advance directives and supportive care.
