Responsibilities

• Field/triage calls and messages from line, providers, patients and families.
• Call patients and families to report labs and provide check-ins.
• Facilitate prior authorizations and prescription management.
• Coordinate with hospice, admissions, and ongoing communication.
• Facilitate scheduling and clinic flow.
• Data collection and tracking to measure program progress and identify needs.
