Qualifications

• BLS certification by American Heart Association (AHA) within 30 days.
• Licensed Occupational Therapist (OT) with Illinois IDFPR.
• Bachelor’s Degree: Occupational Therapy.

Responsibilities

• Performs patient transfers with assistance as needed from bed, chair or floor.
• Uses safe body mechanics to protect staff and patients.
• Designs an individualized plan of care with goals, treatment, outcomes and discharge plan.
• Adjusts care based on patient response and evidence.
• Educates patients, families, caregivers, students and other health care providers.
• Documents evaluations and plans of care in therapy EMR and tracks progress.
