Qualifications

• OT licensed in Kentucky with NBCOT certification is required for OT.
• COTA must hold Kentucky OT assistant license and NBCOT certification as COTA.
• CPR and First Aid certifications are required.
• Willingness to collaborate on a multidisciplinary team and participate in telehealth is preferred.
• Bilingual English/Spanish is a plus.

Responsibilities

• Evaluate and treat children ages 0–21 under supervision as needed.
• Carry out the plan of care with supervising OT for COTA.
• Collaborate on a multidisciplinary team to support the child's development.
• Engage in clinic mentorship and professional growth opportunities.
• Participate in flexible scheduling and telehealth options as available.
