Job summary

Qualifications

• Nursing diploma or associate degree required.
• Bachelor's degree preferred for advancement.

Responsibilities

• Provides direct patient care including assessment, diagnosis, planning, implementation and evaluation.
• Promotes patient and family-centered care and educates on home care needs and medication administration.
• Participates in patient safety, quality and regulatory compliance; engages in professional development.
• Develops and documents plans of care with defined goals in collaboration with the interprofessional team and family or caregiver.
• Assesses patient needs across physical, psychosocial, emotional, cultural aspects; ensures communication of the plan of care across the continuum.
