Qualifications

• Master’s degree in Occupational Therapy and a current Oklahoma license issued by the Oklahoma Board of Medical Licensure and Supervision.
• Level I requires eligibility letter from the Board; permanent license must be obtained before end of probation.
• Level II requires master’s degree and Oklahoma license plus 1 year of experience or advanced degree substitutions.
• Level III requires master’s degree and license plus 3 years of experience or substitutions.

Responsibilities

• Provides early intervention services in child and family’s natural environments.
• Completes assessments and makes recommendations based on developmental indications.
• Develops and implements Individualized Family Service Plans.
• Mentors Occupational Therapy intern fieldwork students.
• Maintains confidential records and participates in team evaluations.
