Job summary

St. Mary's Healthcare System for Children is seeking a part-time Occupational Therapist to evaluate, plan, implement, and document care for home care patients in Uniondale, NY.

You will supervise a Certified Occupational Therapy Assistant (COTA), cosign documentation, and coordinate with the care team to ensure safe, effective home-based therapy. The role requires NY OT licensure and a minimum of one year of pediatric OT experience; base pay is $52,200 per year.

Qualifications

• Registered by the American Occupational Therapy Association or equivalent.
• Licensed in NY as an Occupational Therapist.
• Minimum 1 year pediatric OT experience.

Responsibilities

• Provides skilled occupational therapy in home care per plan of care.
• Supervises COTA and cosigns documentation when required.
• Evaluates patient function and assists in plan of care development.
• Performs home safety assessments and coordinates with the team.
• Documents patient progress and submits weekly visit notes.
• Coordinates with other health professionals for continuity of care.
• Participates in team meetings and quality improvement initiatives.
