Responsibilities

• Interprets physician referrals and evaluates patient abilities and ADLs impact.
• Establishes care plans and implements treatment based on evaluation and goals.
• Provides and fabricates assistive devices to enhance independence.
• Monitors outcomes and adjusts treatment plan with physician input.
• Plans for discharge, teaches home programs, and coordinates community referrals.
• Completes documentation per policy with necessary signatures.
