Job summary

Advocate Health is seeking a licensed Occupational Therapist to join our rehabilitation team. The role supports evaluation, treatment planning, and patient discharge planning in a fast-paced hospital setting.

You will interpret referrals, develop care plans, fabricate assistive devices, and collaborate with physicians to achieve functional goals while maintaining documentation and compliance.

Qualifications

• Master’s Degree in Occupational Therapy from an accredited program.
• Licensed Occupational Therapist in the state of practice.
• No prior experience required for this role.
• Requires strong communication, interpersonal and organizational skills.
• Demonstrates ability to use computers for documentation and scheduling.
• Annual educational requirements for hospital certifications may apply.

Responsibilities

• Interpret physician referrals and evaluate patient abilities and daily living activities.
• Plan and implement individualized treatment plans and supervise assistive devices as needed.
• Monitor outcomes and adjust treatment plans; provide updates to physicians and teammates.
• Discharge planning with home programs and family instruction; coordinate community referrals.
• Document all care and statistics per policy; manage billing and scheduling.
