Founded in 1967, The HealthCare Connection (THCC) was Ohio’s first Federally Qualified Health Center (FQHC). Our mission is to provide quality and accessible primary healthcare services through community responsive approaches that address financial, geographic, and other barriers to care for residents of northern Hamilton County and surrounding areas.
THCC serves over 20,000 patients annually and operates two primary care clinics and six school-based health centers and growing! Our integrated model includes Primary Care, Pediatrics, Infectious Disease, Substance Use Treatment, Integrated Behavioral Health, Dental Services, Women’s Health, and Pharmacy. We are seeking a dedicated and experienced Pediatric Nurse Practitioner (CNP) to join our clinical team. This role works as part of the clinical provider team, delivering a wide range of direct primary care services, including patient assessment, diagnosis, treatment implementation, evaluation, and ongoing monitoring of care plans. The ideal candidate will collaborate with physicians and healthcare staff to provide high-quality, patient-centered care and drive positive health outcomes.
