Lone Star Circle of Care is seeking an APRN to deliver outpatient care to underserved populations in Central Texas. The role supports both in-person and telemedicine visits, emphasizing partnership with patients to decide on treatment plans and improve community health outcomes.
Strong emphasis on coordinating with community partners and EMS/psychiatric services, and documenting care within the electronic health record to maintain continuity of care for patients.

Qualifications: You are a board-certified APRN who is passionate about working with the underserved population who may be experiencing significant barriers to care. You are comfortable providing care both in person and virtually, and you want to grow a practice where you and your patients work together to decide the best course of treatment in achieving their goals in an outpatient setting.
