VCU Health System Authority is seeking a Nurse Patient Navigator to advocate for patients, educate families, and coordinate care with an interdisciplinary team at Children's Hospital of Richmond.

The role requires an RN license in Virginia or eligible/compact state, Baccalaureate in Nursing, and 3+ years of nursing experience with focus on education, case management or navigation. A master’s in nursing is preferred.

Qualifications

• Minimum of three (3) years of relevant clinical experience in nursing, patient education; case management and/or navigation.
• Previous nurse navigation or leadership experience (preferred).
• Experience within an academic, teaching hospital (preferred).
• Bachelor's Degree in Nursing from an accredited School of Nursing.
• Master's Degree in Nursing from an accredited School of Nursing (preferred).

Responsabilités

• Advocate, educator, interpreter, and counselor for assigned patients and caregivers.
• Collaborate with a multidisciplinary team to ensure high quality and comprehensive care.
• Serve as the single point of contact to assigned patients and caregivers.
• Provide ongoing support to patients and families throughout treatment.
• Follow physician/licensed provider orders and practice per Virginia nursing regulations and ANA standards.
