By The Bay Health is seeking a Pediatric Nurse Case Manager to provide skilled nursing assessment, planning, and care for pediatric patients and families in alignment with policies and regulations. This per diem role covers San Francisco with occasional Contra Costa County coverage, and requires California RN licensure and pediatric nursing experience.

Formación
- One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
- One year of pediatric nursing experience
- Experience in acute care/home health and/or hospice setting preferred
Responsabilidades
- Perform skilled nursing assessment, planning and care to maximize comfort and health of pediatric patients and families
- Coordinate with attending physicians, hospice staff and other agencies to optimize patient care
- Maintain regular communication with the attending physician regarding patient needs and responses
- Apply nursing process to assess, plan, implement and evaluate care for pediatric patients
