Qualifikationen

• One year of professional nursing in the past three years or a BSN.
• One year of pediatric nursing experience.
• Experience in acute care/home health/hospice setting preferred.

Aufgaben

• Assess, plan, implement and evaluate pediatric patient/family needs.
• Coordinate care with attending physicians, hospice staff, facility staff and other agencies.
• Maintain regular communication with the attending physician regarding patient/family care needs.
• Apply hospice admission criteria when applicable to determine appropriate levels of care.
