Job summary

Sunset Health in Yuma, AZ seeks a certified Nurse Practitioner to provide primary and ambulatory care services within Center guidelines and in collaboration with the care team.

You will manage a patient panel, coordinate care, educate patients, and support population health initiatives while leading the PCMH team with physicians and other providers. An active AZ NP license, DEA, CPR, and fingerprint clearance are required.

Qualifications

• Current Nurse Practitioner license
• Arizona NP licensure in good standing
• DEA certificate current as required by law
• CPR certification current as required by law
• Fingerprint clearance card

Responsibilities

• Provide professional medical services within privileges approved by the Board of Directors.
• Maintain a patient panel in line with Center standards.
• Coordinate care and manage population health with care team members.
• Lead the patient-centered medical home care team with physicians.
• Establish rapport with patients and others to understand care requirements.
