Job summary

Qualifications

• Master's degree in Social Work (MSW) from an accredited college or university.
• Two (2) years of progressively responsible and directly related work experience.
• Valid license as a Licensed Clinical Social Worker (LCSW) issued by the California Board of Behavioral Sciences preferred.

Responsibilities

• Assess patients and families to evaluate social, emotional and practical needs.
• Provide emotional support, advocacy and development of care plans.
• Collaborate with physicians and healthcare staff to coordinate referrals and services.
• Maintain records and document case activity; prepare reports.
