Job summary

UnitedHealth Group's Minden LA PCS, part of LHC Group, is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes under the supervision of a Registered Nurse.

You will document visits in the electronic medical record, coordinate care with the health team, and support patient needs in the geographic service area. LA licensure, CPR, reliable transportation, and compassionate communication are required.

Qualifications

• Current and unrestricted LPN licensure in state of practice
• Current CPR certification
• Current driver's license and vehicle insurance, access to dependable vehicle or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Responsibilities

• Provides high quality clinical services within the scope defined by state laws and plan of care, in coordination with the health care team
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements
• Acts in care coordination, reporting patient needs and changes in condition, and communicating with patient, family, physician and care coordinators
