UnitedHealth Group's Hammond LA PCS is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes, document visits, and coordinate care under supervision.

The role requires LA LPN licensure, CPR, a valid driver's license, and 1+ year of LPN experience in LA; home care experience preferred. The hourly pay range is $22.90 to $34.35, with benefits and growth opportunities.

Qualifications

• Current and unrestricted LPN licensure in state of practice (LA).
• Current CPR certification.
• Current driver's license and vehicle insurance; access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client characteristics.
• LA state: 1+ years of LPN experience.

Responsibilities

• Provides high quality clinical nursing care in patients' homes as prescribed by the physician and in accordance with the plan of care.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Coordinates care and communicates effectively with patient, family, physician, and care team.
