Job summary

Affirmed Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled pediatric nursing care for a child in Freehold, NJ, in a private duty home setting. This 1:1 role offers flexible scheduling and the chance to form meaningful, long-term relationships with the family.

Responsibilities include G-tube care, PRN oxygen, airway clearance therapies, medication administration, and thorough documentation in collaboration with the pediatric clinical team.

Qualifications

• Active New Jersey LPN license is required.
• G-tube care experience preferred.
• Pediatric home care experience is a plus.
• Current BLS certification is required.
• Authorized to work in the United States.
• Reliable transportation to and from client homes.
• Physical examination within the last 12 months.
• PPD or QuantiFERON TB test within 1 year (or chest X-ray within 5 years).
• Proof of flu vaccine or approved exemption is required.

Responsibilities

• Provide 1:1 pediatric nursing care in the home setting.
• Administer medications and treatments as ordered.
• Monitor and manage G-tube feedings and care.
• Assess respiratory status and provide PRN oxygen support.
• Assist with airway clearance therapies including compression vest treatments.
• Monitor for seizure activity and implement precautions.
• Maintain accurate clinical documentation.
• Collaborate with family, physicians, and pediatric clinical team.
• Support safety, comfort, and quality of life for the child.
