The position supports primary care and behavioral health services, including telehealth and in-office days, with a focus on pediatric and adolescent populations.

Responsibilities

• Embedded within a family medicine office as Integrated Care Clinician
• Work with pediatric and adolescent patients across acute to chronic conditions
• Conduct depression/anxiety and other screenings; consult with medical staff
• Collaborate in a fast-paced interdisciplinary team environment
