Job summary

Global Channel Managementinc is seeking a Social Worker with at least 2 years of post-graduate hospital social work experience to join our team in Chicago. The role requires Illinois LCSW licensure and strong interpersonal skills to support patients and families.

You will assess patient needs, collaborate with interdisciplinary teams, and develop intervention plans. Pediatric-focused experience, especially with diabetes populations, is preferred, and daily caseload management is expected.

Qualifications

• 2 years post-graduate hospital social work experience.
• High level interpersonal and organizational skills to manage caseloads.
• Self-direction required for daily work.
• Analytical ability to collect, analyze, and interpret data and negotiate in sensitive situations.
• Illinois LCSW licensure is required.
• Experience with pediatric populations.
• Experience with Type 1 and Type 2 diabetes populations preferred.

Responsibilities

• Meets directly with patient/family to perform a comprehensive assessment including social, emotional, cultural, mental status, environmental and financial circumstances in conjunction with interdisciplinary assessment of the patient.
• Recommends a plan of intervention based on patient needs, preference and mutually established goals.
