Zusammenfassung

The Staff Pad is partnering with a respected hospice provider in the DFW Metroplex to recruit a compassionate Pediatric RN Case Manager. You will deliver skilled nursing care to pediatric hospice patients in their homes while coordinating all aspects of care with an interdisciplinary team.

This full-time, field-based role involves local travel, on-call rotations, and collaboration with physicians and community partners to ensure dignified, patient-centered care for children and families.

Qualifikationen

• Graduate of an accredited Diploma, ADN, or BSN Nursing program.
• Current Texas Registered Nurse (RN) license
• Minimum one (1) year of pediatric RN experience in a clinical setting required
• Hospice, Critical Care, or Emergency Room experience preferred
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Current TB testing and Hepatitis consent/declination documentation
• Ability to pass a criminal background check

Aufgaben

• Conduct comprehensive physical, psychosocial, and environmental patient assessments
• Develop, implement, and document individualized plans of care
• Communicate effectively with patients, families, physicians, and the care team
• Provide clinical guidance to Hospice Aides and LVNs and assist with training as needed
• Complete pediatric admissions and prioritize patient scheduling
• Participate in assigned on-call rotations for emergency patient support
• Maintain accurate documentation and ensure compliance with infection control and agency policies
• Support ongoing quality improvement and professional development
• Perform other duties as assigned
