BrightStar Care is seeking a compassionate Pediatric Home Health RN (PRN) to provide one-on-one nursing care for medically fragile children in their homes across Texas. You will implement physician plans, monitor patient conditions, and educate families to ensure safe, high-quality care.

The role offers flexible PRN scheduling with travel within the service area. Previous pediatric home health experience is preferred, and a valid Texas RN license is required.

Qualificações

• Current, unrestricted Registered Nurse (RN) license in Texas.
• Current CPR/BLS certification.
• Minimum 1 year of nursing experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Reliable transportation and valid driver's license.

Responsabilidades

• Provide skilled nursing care to pediatric patients in the home setting.
• Perform comprehensive nursing assessments and ongoing evaluations.
• Administer medications and treatments as prescribed.
• Monitor and document patient responses to interventions.
• Recognize changes in patient condition and promptly notify appropriate healthcare providers.
• Follow and implement individualized Plans of Care (POCs).
• Communicate effectively with physicians, therapists, case managers, and caregivers.
• Participate in care conferences and interdisciplinary team collaboration as needed.
• Foster a supportive, family-centered approach to care.
• Complete accurate and timely clinical documentation.
• Maintain compliance with agency policies, state regulations, and nursing standards.
• Follow HIPAA and patient confidentiality requirements at all times.
