BrightStar Care is seeking a compassionate and skilled Pediatric Home Health Registered Nurse (RN) PRN to provide high-quality, one-on-one nursing care to medically fragile children in the comfort of their homes. The Pediatric Home Health RN is responsible for implementing physician-ordered care plans, monitoring patient conditions, providing family education, and ensuring the highest standards of clinical excellence and patient safety.
This position offers flexibility while allowing nurses to make a meaningful difference in the lives of pediatric patients and their families.
Key Responsibilities
Direct Patient Care
- Provide skilled nursing care to pediatric patients in the home setting.
- Perform comprehensive nursing assessments and ongoing evaluations.
- Administer medications and treatments as prescribed.
- Monitor and document patient responses to interventions.
- Recognize changes in patient condition and promptly notify appropriate healthcare providers.
Specialized Pediatric Care
- Provide care for patients with:
- Tracheostomies
- Mechanical ventilators
- Gastrostomy tubes (G-tubes)
- Seizure disorders
- Oxygen therapy
- Other complex medical needs
Ensure all medical equipment is functioning properly and safely.
Care Coordination
- Follow and implement individualized Plans of Care (POCs).
- Communicate effectively with physicians, therapists, case managers, and caregivers.
- Participate in care conferences and interdisciplinary team collaboration as needed.
Family Education & Support
- Educate families and caregivers regarding:
- Disease processes
- Medication administration
- Equipment use
- Emergency procedures
- Foster a supportive, family-centered approach to care.
Documentation & Compliance
- Complete accurate and timely clinical documentation.
- Maintain compliance with agency policies, state regulations, and nursing standards.
- Follow HIPAA and patient confidentiality requirements at all times.
Qualifications
Required
- Current, unrestricted Registered Nurse (RN) license in Texas.
- Current CPR/BLS certification.
- Minimum 1 year of nursing experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Reliable transportation and valid driver's license.
Preferred
- Pediatric nursing experience.
- Home health or private duty nursing experience.
- Experience caring for medically complex pediatric patients.
- Familiarity with EMR/EHR documentation systems.

Working Conditions

From $40.00 per hour
