UnityPoint Health is seeking a Home Care Nurse to provide skilled nursing in patient homes, including assessments, medications, feedings, and respiratory support. You will collaborate with RN Case Managers and clinicians to deliver safe, compassionate care under physician direction.

The role emphasizes patient education, documentation, and coordination with the care team, with a $10,000 sign-on bonus and a Monday–Friday 7:30 AM–4:30 PM shift.

Qualifications

• RN license in state(s) of practice required.
• BSN preferred; Masters preferred.
• Experience in clinical healthcare and pediatric home care preferred.

Responsabilités

• Deliver high-quality home health care in alignment with physician orders, care plans, and regulatory and payer requirements.
• Coordinate with RN Case Managers and clinical supervisors to ensure safe, effective, patient-centered care delivery.
• Conduct skilled visits and monitor patient status, contributing to ongoing assessment and updates to the plan of care.
• Educate patients, families, and caregivers on diagnoses, safety, and self-management to support better outcomes.
• Maintain accurate, timely clinical documentation and communication, including EMR updates and required reporting.
• Collaborate with interdisciplinary team members to support comprehensive care planning and follow-up.
• Participate in onboarding and training of new staff and contribute to a learning environment.
• Respond promptly to communications and engage in required learning activities.
• Support operational efficiency by completing assigned duties and adapting to evolving needs.

Shift: Monday - Friday | 7:30 AM - 4:30 PM

Make a meaningful impact in the lives of pediatric and young adult patients with complex health care needs as a Home Care Nurse. In this role, you will provide skilled nursing care in the home, including patient assessments, medication administration, feedings, respiratory support, and assistance with daily living and therapy programs. Working under physician direction, you will collaborate with the care team to support coordinated care and may also assist with patient transportation.

Qualifications

Education:

• Graduate of State Board approved program for Registered Nursesrequired.
• BSN or BS in health related fieldpreferred.
• Masters education in health relatedpreferred.

Experience:

• Experience working in clinical healthcare position preferred.
• Previous pediatric home care experience preferred.

License(s)/Certification(s):

• Valid RN license in the state(s) of practice, including eligibility under a NLC multistate license required.
• Dual licensed positions must obtain and submit proof of dual licensure within 90 days of hire/transfer; proof of application is acceptable during this period.
• Valid Drivers license andMaintenance of an acceptable driving record (i.e., no "OWI" convictions in last 7 years or no more than 3 moving violations in last 3 years)required.
• Valid Auto Insurance required when driving personal vehicle.
• Basic Life Support (BLS) certification required within 30 days through the American Heart Association.
• IA Child Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.
• IL Child Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.
• IA Dependent Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.
• IL Dependent Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.

Signing bonus eligibility is based on role requirements and assignment. Signing bonuses may not apply to positions involving care for a family-member patient.
