You will provide skilled nursing care, monitor patient conditions, manage medications and therapies, and support families through education and triage, ensuring safe, high-quality care.

Responsibilities

• Provide skilled nursing care in home and school settings
• Assess and monitor your patient's condition and communicate changes with the care team
• Administer medications, treatments, and prescribed therapies
• Provide trach, ventilator, airway, and G-tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
