Qualifications

• Active nursing license (LPN or RN) in Minnesota.
• Experience with pediatric home care preferred.
• Ability to manage complex pediatric conditions in home/school settings.

Responsabilités

• Provide skilled nursing care in home and school settings
• Assess and monitor your patients condition and communicate changes with the care team
• Administer medications treatments and prescribed therapies
• Provide trach ventilator airway and G tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
