• Provide skilled nursing care in home and school settings
• Assess and monitor patient condition and communicate changes with the care team
• Administer medications, treatments and prescribed therapies
• Provide trach, ventilator, airway and G-tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
