Nationwide Children's Hospital in Columbus, OH seeks a Registered Nurse, Care Coordinator in Hematology to ensure quality patient care through care coordination, case management, utilization review of admissions, and smooth transitions of care.

You will collaborate with physicians, families and the health care team, develop individualized plans of care, educate families, and coordinate post-discharge arrangements. A BSN and Ohio RN license are required; pediatric experience is preferred.

Qualifications

• Communication with health care professionals with diplomacy and tact.
• Effective collaboration and delegation of tasks within a team.
• Working knowledge of CMS and related standards.
• Ability to relate to diverse age and demographic backgrounds.
• Proficiency in MS Office and data interpretation.

Responsabilités

• Screens and identifies patients and assesses family strengths and needs.
• Develops a family-centered plan of care.
• Facilitates communication among the health care team; ensures follow-up across the care continuum.
• Educates patients and families to build self-management skills and resources.
• Manages transitions between settings and coordinates post-discharge care.
• Keeps current with care coordination concepts, case management and discharge planning.
