Job summary

Nationwide Children's Hospital in Columbus, OH, seeks a Care Coordination Nurse to ensure quality patient care through care coordination, case management, and transitions to different levels of care within hospice and home-based palliative services.

Requires BSN, Ohio RN licensure, and at least five years of nursing experience, with pediatric experience preferred; this full-time role features a Monday–Friday schedule, 8am–5pm, plus comprehensive benefits and a $5,000 sign-on bonus.

Qualifications

• BSN is required.
• Current licensure as an Ohio RN is required.
• Five years of nursing experience is required, with pediatric experience preferred.

Responsibilities

• Assesses patient/family needs and develops a family-centered plan of care with the interdisciplinary team.
• Manages transitions between settings and coordinates care among providers and hospice teams.
• Participates in inpatient care conferences and supports education on care coordination.
