Job summary

Qualifications

• BSN required.

Responsibilities

• Screen and identify patients.
• Assess patient and family needs.
• Develop family-centered plan of care.

• Facilitate transitions between care settings.
• Maintain knowledge of care coordination concepts.
• Educate others on care coordination roles.
• Engage in IT/system requirements for the department.
