Responsibilities

• Demonstrates understanding of organizational goals within quality and safety performance initiatives
• Collects, assesses and documents patient health data
• Identifies realistic, measurable plans of care with the interdisciplinary team
• Prioritizes care based on data and plan of care
• Documents assessments and interventions in medical records
• Educates patients and families about health status and the care plan
• Evaluates progress toward expected outcomes with patient/family involvement
• Maintains compliance with NY state nurse practice act and regulatory requirements
