Qualifications

• BSN is required for licensed practice.
• At least 3 years of inpatient/outpatient nursing experience.
• Excellent communication skills required.
• Ability to de-escalate and perform service recovery.
• Knowledge of clinical processes and hospital procedures for admission, inpatient stay, transfer, discharge and/or outpatient/ambulatory setting.

Responsibilities

• Review data and coordinate with all involved teams to meet patient and family needs during inpatient stay or outpatient setting.
• Identify and prioritize clinical and psychosocial goals throughout inpatient stay or outpatient setting.
• Ensure optimal inpatient or outpatient coordination and focus on family experience and outcomes.
• Collaborate with social work, case management, language services, and other support services.
• Document conversations, rounds, and referrals; update leadership as needed.
