Qualifications

• DDS or DMD accredited by the Commission on Dental Accreditation
• Certificate in Pediatric Dentistry
• Valid license to practice dentistry in the state of care; in good standing or eligible for licensure
• Active state-specific anesthesia license (for nitrous oxide administration)
• Additional certifications as required by state: DEA, CPR, PALS
• No sanctions related to Medicare, Medicaid, or licensing boards
