Responsibilities

• Assess comprehensive data across physical, psychosocial, emotional, cognitive, and spiritual needs.
• Involve patient, family and health care team in formulating an individualized plan of care and identify expected outcomes.
• Implement the plan of care in partnership with the patient, family and health care team members.
• Evaluate the patient/family's understanding of and response to the plan of care.
