Qualifications

• Experience with transitions of care and patient-centered pharmacotherapy.

Responsibilities

• Provide pharmacotherapy expert guidance to optimize outcomes.
• Document care and order verifications in the medical record.
• Collaborate with multidisciplinary team for patient-centered care.
• Precept pharmacy residents, students, or interns.
• Participate in quality improvement and safety initiatives.
