Nationwide-Children Hospital in Columbus, OH seeks an experienced pharmacist to provide pharmacotherapy services across pediatric and select adult populations. You will integrate direct patient care, medication safety, transitions of care, and collaborative care to optimize outcomes from birth through 21 years.

The role includes teaching and precepting, participation in quality improvement, and the supervision of residents and students.

Qualifications

• Doctor of Pharmacy, required.
• Licensed or licensure-eligible to practice in Ohio.
• BPS Board Certification or eligible with certification to be obtained within 24 months of hire.
• PGY1 residency with PGY2 or fellowship, or 5 years hospital pharmacist practice with pediatrics focus.

Responsibilities

• Provide pharmacotherapy recommendations and optimize drug therapy with safety and efficacy.
• Verify orders and ensure timely, accurate medication delivery.
• Maintain accurate medication lists and communicate with the multidisciplinary team.
• Precept residents, students, interns, technicians, or graduates awaiting licensure.

Advances care through teaching and precepting, quality improvement, and professional scholarship.Job Description:Essential Functions:Serves as a member of the patient care team as the pharmacotherapy expert to prospectively recommend and evaluate individualized drug therapy, makes recommendations to achieve best outcomes in an evidence-based and patient-specific manner while considering pharmacoeconomic principles, and prospectively identifies potential adverse drug reactions, drug-drug, disease and drug-food interactions, and incompatibilities affecting patient outcomes.Documents direct patient care, clinical, operational, and financial outcomes and activities appropriately in the medical record; operationalizes recommendations through order verification to ensure timely and accurate medication delivery in a form appropriate for the patient.Focuses on accurate maintenance of the medication list, ensures accurate and timely communication to the multidisciplinary team, other pharmacists, and patient/family regarding medication-related needs; and participates in the development and coordination of patient-specific teaching and transitions of care services.Provides appropriate handoffs for pharmacists and other healthcare providers to follow up on anticipated pharmacotherapy needs, including medication changes. Collaborates with other members of the pharmacy team to assure continuity of care and consistency of practice.Participates in hospital and/or department committees as related to medication safety and /or quality improvement initiatives while demonstrating pharmacy practice leadership.Provides in-service training to medical, nursing, and pharmacy staff. Precepts and/or supervises pharmacy residents, students, interns, technicians, or graduates awaiting licensure. Provides appropriate oversight and input in performance evaluations.
