Job summary

Central California Alliance for Health is seeking a dedicated Case Management Nurse for a temporary full-time role in California. You will develop individualized care plans, educate families and providers, and collaborate with internal teams to improve outcomes.

The role emphasizes quality improvement, cost-effective care, and coordination across the care continuum. Hybrid work is expected, with some onsite presence and remote interviews via Teams.

Qualifications

• Current unrestricted RN license issued by the State of California.
• Associate's degree in Nursing with at least five years' patient care experience, including case management.
• Experience in acute or outpatient settings with pediatric population preferred.

Responsibilities

• Develops and manages an individualized comprehensive plan of care for members referred into the Case Management Program to promote optimal, achievable outcomes in a cost effective and appropriate manner.
• Educates members, families, providers, external agencies and internal departments about the Case Management Program and provides necessary assistance.
• Participates in Quality Improvement studies to continually evaluate the program’s effectiveness and ability to promote quality driven, cost effective goals and outcomes.
