Job summary

Children's Wisconsin seeks a dedicated Care Coordinator to manage care coordination for children with medical complexity. You will partner with nurses and clinicians to connect families with clinical, community, and social resources, ensuring follow-up and adherence to medical plans.

You will maintain documentation in electronic health records, triage calls, and schedule tests while collaborating with social work and family services to optimize outcomes for pediatric patients and their families.

Qualifications

• Proficient data entry skills with accuracy and speed.
• Knowledge of the needs of low-income families and coping styles of families under stress.
• Ability to navigate, coordinate multiple services, prioritize and multi-task.

Responsibilities

• Facilitates continuity of health care by coordinating with providers and agencies while preserving confidentiality.
• Links families to local services and documents all communications and services.
• Acts as a liaison between families, providers, schools, nursing agencies and community resources.
