Job summary

NYC Health + Hospitals Queens Hospital Center is seeking a compassionate Community Health Worker to support Pediatric Primary Care. You will educate families, coordinate access to primary and specialty care, behavioral health services, and social supports, and help families navigate health systems.

The role involves team collaboration, home visits, and documenting encounters in the electronic medical record. NYC residency is preferred and strong communication skills are essential.

Qualifications

• Three (3) years of full-time experience in counseling, community work or community health activities.
• Satisfactory combination of education and/or experience equivalent to 1 above; at least 1 year of full-time experience if substituted.

Responsibilities

• Engage patients and families to identify health goals and needs.
• Coordinate care by connecting patients to primary and specialty care, behavioral health, and community resources.
• Document each patient encounter in the electronic medical record and use EHR systems per policy.
• Attend regular team meetings and communicate with patients’ PCPs.
• Conduct home and community-based visits to assess needs in the home environment.
• Serve as liaison between the community and health system and report to CHW Supervisor.
