Responsibilities

• Educate and counsel patients, families, and healthcare team on safe medication use.
• Participate in safe, timely, and cost-effective medication therapy and stewardship.
• Engage in direct patient care and interdisciplinary rounds; ensure smooth transitions of care.
• Supervise and mentor pharmacy staff and learners.
• Identify and report medication safety events and errors.
• Utilize EHRs and pharmacy informatics to optimize decisions and documentation.
• Contribute to practice advancement and research.
• Perform all pharmacist functions including compounding, dispensing, verification, and documentation.
• Review orders and patient profiles to resolve drug-related problems; adjust dosing as needed.
• Participate in quality improvement and regulatory activities.
