Job summary

HCA Healthcare Trident Hospital in Summerville, SC seeks a Pediatric Clinical Pharmacist to provide clinical pharmacy services in alignment with regulations and hospital policies. You will apply pharmacology knowledge to patient care in dosing, monitoring, and therapy optimization.

Responsibilities include interpreting orders, accurate compounding and dispensing, anti-interaction monitoring, and collaboration with physicians and nurses.

Qualifications

  • ACPE-accredited School of Pharmacy graduate; PharmD preferred.
  • Active Pharmacist license in the practicing state required.
  • BCPPS certification preferred or to be obtained within 6 months of hire.
  • Minimum 1 year hospital pharmacy experience required.
  • Prior hospital pharmacy residency preferred.

Responsibilities

  • Interpret orders and transcribe/verify in the pharmacy computer system efficiently.
  • Accurately compound and dispense pharmaceuticals.
  • Monitor drug therapy regimens for contraindications, drug–drug and drug–food interactions, allergies and appropriateness of dose.
  • Consult with physicians and nursing staff on drug availability, dosage form, side effects and compatibility; suggest cost-effective alternatives.
  • Sustain the formulary, minimize nonformulary procurements and promote rational therapy.
  • Provide accurate medication information to healthcare staff and patients.
  • Maintain controlled substance records.
  • Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs.

Job Summary

The Pediatric Clinical Pharmacist is responsible for providing general clinical and pharmacy services in compliance with regulatory and hospital policies/procedures. The Pharmacist will provide patient care through the use of knowledge, skills and abilities in compounding and dispensing pharmaceuticals. Responsibilities
  • Interpret orders and transcribe/verify in the pharmacy computer system efficiently.
  • Accurately compound and dispense pharmaceuticals.
  • Monitor drug therapy regimens for contraindications, drug‑drug interactions, drug‑food interactions, allergies and appropriateness of drug and dose.
  • Consult with Physicians and nursing staff concerning the availability, dosage form, side effects, and possible incompatibility of new and currently used drugs. Suggest appropriate, cost‑effective therapeutic medication alternatives to medical staff.
  • Sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy selection, limiting unnecessary procurement costs.
  • Provide accurate and timely medication information to healthcare staff and patients.
  • Maintain appropriate controlled substance records.
  • Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
Qualifications
  • Graduate of ACPE-accredited School of Pharmacy is required; PharmD Degree is preferred.
  • An active Pharmacist license with the Board of Pharmacy in the state in which you practice, in accordance with law and regulation is required.
  • BCPPS is preferred but required within 6 months of hire.
  • Minimum of 1 year of hospital Pharmacy experience is required.
  • Prior hospital Pharmacy Residency is preferred.
