Qualifications

• Current RN license in state of practice.
• BLS certification for healthcare providers.
• BSN preferred; RN with <12 months must obtain BSN within four years.

Responsibilities

• Assess physical, social and psychological status of patients.
• Collaborate with interdisciplinary team to plan individualized care.
• Provide education to patients and families related to care and follow-up.
• Document care, outcomes and adherence to care protocols.
