Responsabilités

• Coordinate the scheduling of client visits and maintain scheduling records.
• Communicate scheduling changes between office and field staff in a timely manner.
• Act as a liaison between clients and scheduled staff to communicate needs and changes.
• Deliver superior customer service to clients and field staff for high-quality home care.
• Manage coordination of client services and emergent scheduling issues.
• Build relationships with clients, referral sources, payers, and community organizations.
• Handle a caseload and support management initiatives while meeting deadlines.
• On-call availability after work hours.
